Do ChampionsTX offer discounts? YES! DISCOUNTS SIBLING DISCOUNT: 10% off per class for any child after the first enrollment. The 10% will be applied to the tuition of lower value. Applies to classes/tuition only. Camps, and Special Events will not include a sibling discount MULTI CLASS DISCOUNT: 10% off per class for any class after the first enrollment. SCHOLARSHIPS: Every year, ChampionsTX awards students with scholarships towards their tuition at ChampionsTX. While we do not require financial information to apply, this is done on the honor system in an effort to reserve the scholarships for families who need assistance. If spaces become available mid-season, families put on the waitlist will be contacted. To apply, email waya@ChampionsTX.com. 2ND GENERATION CHAMPION DISCOUNT: If you were once a ChampionsTX athlete or a coach and are now bringing your child to us then welcome to the 2nd Generation of ChampionsTX Club. We would like to gift our 2nd Generation Champions a free Leotard or T-Shirt and your first month of classes free! Please contact our office and we will help you find the perfect class and apply your gift to your account! This offer also extends to our amazing 3rd generation families!

Do ChampionsTX offer free trial classes? YES! Even though all classes follow the same progression lists, due to the makeup of kids enrolled in classes, all classes have a slightly different feel, and your child might enjoy and do better in one class more than another. If you prefer, you can schedule an evaluation with our floor manager to get a class recommendation and find the perfect class match! Free Trial Classes are limited to 1 per child per season. Free trials must be booked in advance by registering through your parent portal, emailing, or calling the office. You cannot do a trial in a full class or a class with a waitlist.

Can I watch my childs class? YES! PARENT WATCH POLICY: Parents are welcome to watch their child anytime in our parent watch area. However, for safety reasons and in order not to distract the students, siblings who aren’t enrolled should remain off the mats and equipment. There is a kids play area available for active toddlers. Please clean up toys and games after your children. We also ask that all parents stay off the gym floor and any mats for safety/liability reasons if choosing to watch your athlete(s) as surfaces around the gym are uneven and athletes rotate around the gym. We also offer official parent watch days throughout the year in which you will be allowed out on the floor – including an end-of-year award ceremony in May.

Does ChampionsTX offer make up classes? YES! We are proud to be one of the few gyms to offer makeups! Due to full classes and in an effort to uphold our class quality for your child, we will no longer be allowing make ups during normal classes. Instead we will be doing make up classes once a month on the last Friday of each month at 5:30 pm and on the last Saturday of each month at 12:45 pm. We will be hosting a Gym Kids class (good for kiddos 5 and under), a Gymnastics class (good for kids 5- 12), a parkour class (Saturdays only- good for kids 5- 12), and a tumbling class (Saturdays only- good for kids 5- 15). You must call to get registered for that make-up class. You may only receive one make up per month and that make up must be used the month the class was missed. Make ups do not roll over to the next month. If you have any questions, please feel free to email or call us! During the summer months, our make up policy is different. We do allow for make ups during the week as our summer classes don’t tend to be as full. Does ChampionsTX prorate tuition? YES! We do prorate tuition if you join in the middle of a month! MISSED CLASSES: Class tuition reserves a spot in the class your child is enrolled in, and therefore tuition is not based on attendance. If your child misses eight classes in a row and we have not been able to reach you, your child will be removed from the class. This opens up a spot for other students on the waitlist and protects you from being charged further for a class each month that you are not attending. CALENDAR & GYM CLOSINGS: Follow the calendar on our website for closures, special events, and other dates. You do not pay for any planned closures, such as Winter Break, as they are not calculated into tuition. ChampionsTX generally follows the school district/ WAYA decisions in the case of inclement weather. Check Instagram @championstx_waya for up to date weather closings. What should my athlete wear to class? Girl gymnasts should wear a leotard. We do have leotards for sale in the gym. Boys should wear a t-shirt and athletic shorts. Tumble and cheer students should wear a fitted tank top and athletic shorts. Cheer students must wear cheer shoes for safety during stunting. Important Notes: Dresses or skirts are highly discouraged for athletes. If a student chooses to wear one, fitted shorts are required to be worn underneath. Changing clothes is only permitted inside the bathrooms downstairs. Hair must be pulled neatly and securely away from the face so it stays out of the eyes. The following are not appropriate for the gym: jeans, belts, zippers, skirts/ dresses without shorts underneath, jewelry, socks, sports bras without a fitted shirt overtop, or footed tights. Athletes not dressed appropriately for class or team will be asked to change before participating.

ChampionsTX Policies

Champions Academy, Inc. Rules, Terms & Conditions Welcome to ChampionsTX! We’re so excited to have your family in our program. To ensure that every athlete and family enjoys a safe, positive, and productive experience, please review the following rules and policies carefully. By enrolling, you acknowledge that you have read and agree to the terms below. GENERAL GUIDELINES Parent Watch Policy Parents are welcome to observe classes anytime from our Parent Watch Area. For safety and insurance reasons: Parents, guardians, and visitors must remain off the gym floor and equipment unless invited by a coach during an official Parent Watch Day or event. Siblings not enrolled in class must stay off all mats and equipment. A Kids Play Area is available for young siblings- please supervise your children and tidy up toys after use. Surfaces in the gym are uneven, and athletes rotate quickly, so please remain in designated viewing areas. Special Parent Watch Days and our end-of-year Awards Ceremony in May allow families to join their athletes on the floor for celebration and photos. Missed Classes Tuition reserves your child’s space in class, which means tuition is not based on attendance. If a child misses eight consecutive classes and we are unable to reach you, we will unenroll your child from the class to: Open the spot for another student, and Prevent further tuition charges for unattended classes. Make-Up Classes ChampionsTX is proud to be one of the few gyms that offers make-up opportunities! To maintain class quality and safety, make-ups will be held in dedicated sessions rather than regular class times. School Year Make-Ups: Held monthly on the last Friday (5:30 PM) and last Saturday (12:45 PM) of each month. Available sessions include: Gym Kids (ages 5 & under) Gymnastics (ages 5–12) Parkour (Saturdays only, ages 5–12) Tumbling (Saturdays only, ages 5–15) You must call ahead to register for a make-up session. One make-up per student, per month. Make-ups must be used within the same month as the missed class and do not roll over. Summer Make-Ups: During summer sessions, make-ups may be scheduled during the week, as class sizes are typically smaller. All summer make-ups expire at the end of the summer session and cannot be carried into the school year. Calendar & Closings Stay up to date on all closures, events, and holidays by checking our calendar at ChampionsTX.com or on Instagram @championstx_waya. Tuition is not charged for scheduled closures (such as Winter Break). In the event of inclement weather, ChampionsTX typically follows WAYA and local school district decisions. Competitive teams follow separate calendars- please check your Band App or email WAYA@ChampionsTX.com for details. ARRIVAL & PICK-UP PROCEDURES Please send your child with a water bottle and hair pulled back securely. Arrive no earlier than 5 minutes before class. Encourage restroom and water breaks before class to maximize instruction time. Upon arrival, check in using the iPad system by entering your phone number. Athletes must remain in the designated waiting area until called onto the floor by their coach. Athletes should only be on equipment when accompanied by a coach. Parents must come inside for pick-up to ensure safe release of all athletes. Late Pick-Ups: Please be on time for pick-up. A 10-minute grace period is allowed for unforeseen delays. Families with 2 or more late pick-ups will be charged a $1.00-per-minute late fee after the third incident. All pick-ups take place upstairs at the cubbies. Please drive slowly and cautiously in the parking lot- many of our students are young children. FACILITY POLICIES Bathroom & Lobby Per WAYA policy, children under age 8 may not remain in the lobby unattended. Please take your child to the restroom before class. A water fountain is available in the lobby, and bottled water can be purchased from the Pro-Shop or vending machines downstairs. Behavior Policy At ChampionsTX, we believe in creating a positive, respectful, and encouraging environment for all. Our coaches will: Give a verbal warning for minor misbehavior. If needed, ask a guardian to join the athlete on the floor for support. If behavior persists, a director may discuss a plan with the parent or temporarily remove the athlete. For camps: Counselors use positive tools and strategies to redirect behavior. If challenges continue, parents will be notified to create a plan. As a last resort, campers may be sent home without refund. Lost & Found ChampionsTX is not responsible for lost, stolen, or unclaimed items. Please label all belongings clearly with your child’s first and last name. Lost items are stored in the Lost & Found bin near the Pro-Shop. Items are donated to charity at the end of each month. If labeled, we’ll do our best to contact you before donation. Food Policy ChampionsTX is a nut-free facility. Please do not bring any nut products into the building out of consideration for those with allergies. Water bottles are encouraged at every class and event. What to Wear Girls: Leotard (shorts optional). Boys: T-shirt and athletic shorts. Tumble/Cheer: Fitted tank or tee with athletic shorts. Cheer students must wear cheer shoes for safety. Hair must be pulled back neatly and away from the face. Not allowed: jeans, skirts, zippers, belts, jewelry, socks, or footed tights (for safety reasons). Cell Phones & Media Use Siblings are welcome to bring iPads, books, or quiet toys while waiting. Devices must be on silent or use headphones. Please keep long or loud phone calls downstairs to avoid disrupting classes. Sick Policy To help keep our community healthy: Children must be fever-free for at least 24 hours (without medication) before attending class. If your child misses school due to illness, they should not attend ChampionsTX that day. Please schedule a make-up if your child has severe cold symptoms, an unknown rash, or lice. If a household member tests positive for COVID, please contact the front desk for guidance. Champions Academy, Inc. Code of Conduct At ChampionsTX, our mission is to create a safe, respectful, and encouraging environment where children and families can grow in skill, confidence, and character. This Code of Conduct outlines the expectations for staff, coaches, athletes, parents, caregivers, and visitors so that every member of our community experiences a positive and supportive atmosphere. By participating in ChampionsTX programs or entering our facility, you agree to uphold these standards. 1. Respect and Sportsmanship Treat all athletes, coaches, staff, parents, and visitors with kindness, patience, and respect. Demonstrate good sportsmanship at all times- cheer for others, celebrate effort, and support your teammates. Speak and act in a manner that reflects the ChampionsTX core values: Respect, Trust, Teamwork, and Forthright Communication. Discriminatory, disrespectful, or inappropriate language or behavior of any kind will not be tolerated. 2. Inclusivity and Positive Environment ChampionsTX welcomes individuals of all backgrounds, abilities, and identities. Bullying, teasing, gossiping, or exclusionary behavior- by athletes, staff, or parents- will result in disciplinary action. Encourage one another and help create a culture where everyone feels valued and supported. Parents and caregivers are expected to model the same positive behavior we expect from athletes. 3. Health, Safety, and Facility Care Follow all safety instructions and spotting guidelines given by ChampionsTX staff and coaches. Only enter activity areas or equipment zones when invited or instructed by staff. Parents, siblings, or caregivers who enter the gym floor must follow all safety rules and remain under staff direction. Report any damaged equipment or unsafe conditions immediately to a coach or front desk staff. Keep the facility clean- return equipment to its proper place and respect shared spaces. Please stay home when exhibiting any illness symptoms (such as fever, cough, or stomach distress) to protect others’ health. 4. Communication and Cooperation Maintain open and respectful communication with coaches and staff. Address concerns privately and professionally- never during a class, practice, or in front of athletes. Parents are encouraged to email or schedule a meeting to discuss progress or concerns. Coaches and staff will strive to communicate clearly, respectfully, and in a timely manner. 5. Commitment to Excellence Athletes are expected to arrive on time, dressed appropriately, and ready to participate with a positive attitude. Coaches and staff will provide instruction that promotes growth, discipline, and confidence in every athlete. Parents and caregivers play a vital role in supporting consistency, encouragement, and good character both inside and outside the gym. ChampionsTX is a team community- each of us contributes to the overall success and experience of all members. 6. Compliance and Consequences All members must adhere to ChampionsTX rules, safety policies, and staff instructions at all times. Behavior that endangers others, disrupts classes, or violates this Code of Conduct may result in: Verbal or written warning Suspension from class or program Permanent dismissal from ChampionsTX programs or facility Our Commitment By following this Code of Conduct, we ensure ChampionsTX remains a place where every child and family can learn, play, and grow in a safe, uplifting, and respectful environment. Champions Academy, Inc. Camp Cancelation Policy Staff is hired and supplies are bought and prepared based on the number of enrolled children in each age group. In order to prepare a first-rate out of this world camp experience for your child, ChampionsTX devotes an extraordinary amount of time and effort to plan and purchase age-appropriate crafts, activities and staff with each specific child in mind. On that note, we hope you will understand that our cancellation policy is as follows: Payment is due in full at the time of enrollment and is non-refundable. No refunds are given for camp. If a request to reschedule is received at least 7 days in advance before the reserved camp begins, we will be happy to change your camp dates. Siblings are welcome to use camp days/weeks. If a child wakes up ill on the day they are scheduled for camp, a doctor’s note must be provided. If your child or a member in your household gets COVID, we will be happy to move your child to another camp. You may use the week you missed in the alternate week. Requests to transfer camps must be received in writing 7 days before the camp start date (of which you are currently enrolled) to waya@ChampionsTX.com. Request to transfer less than 7 days before the start of camp will be charged for one day of camp in addition to the $35 transfer fee. All transfers are subject to availability. Makeups/refunds are not offered for missed camp days. If you choose to cancel your camp and not transfer to another camp you must do so 14 or more days in advance of that camp in order to receive a full credit. If you cancel 7- 13 days in advance, you will receive a credit for half. If you cancel less than 7 days in advance you will be unable to receive a credit.

Champions Academy, Inc. Class Cancelation Policy Class Cancellation Policy: All tuition paid is non-refundable. In order to drop from a class or program, we need written notice to waya@championstx.com 14 days before the 1st. If it is after 14 days, we will be unable to drop you until next month. If you miss the deadline to drop you will be charged your monthly tuition at the time of the drop request and it will stay on your account as a credit to use for any Champions activity (classes, camps, birthday parties) or in the pro-Shop or we can drop you for the following month. Teams are considered a full-year commitment because of our team bond, close friendships, skill progressions, and year of program events, so please consider your decision to join one of our higher-level workouts carefully before accepting your team invitation. You will be automatically dropped at the end of the school-year season (May). Immediate Drop Option: In the event that your child sustains an injury that prevents them from participating (by Dr. orders) for an extended period of time, please contact our office at waya@championstx.com with a doctor’s note. We will then be able to drop you immediately. Dropping the class will not hold your spot in the class. We will be happy to re enroll you once your child is healed but we cannot guarantee they will still have a spot when they return. If you want to ensure that your child keeps a spot you will have to continue to pay monthly tuition while your child is out. Transferring Classes: Let us help you find the PERFECT replacement spot for your Champion! Transfer requests must be made through the office to ensure the correct skill level and availability.

Champions Academy, Inc. Birthday Party, Super Saturday, and Open Gym/ P.O.P Policy Party Policy: Staff is hired based on the party package you book. Once a date has been secured no refunds or credits will be given. We will move your date for unforeseen Illnesses and accidents. If you notify us of a cancellation with less than a 24 hours of notice, we will not be able to move your party date. Super Saturday Policy: If you notify us of a cancellation up to 7 days out from your scheduled date, you will receive a full credit to your ChampionsTX Account. If you notify us of a cancellation with more than 24 hours out from your scheduled date, you will receive a 50% credit to your ChampionsTX Account. If you notify us of a cancellation with less than 24 hours of notice, we will be unable to give credit. If you would like to transfer to another Super Saturday we can transfer you with at least a 24 hours notice. All transfers are subject to availability. All kids attending Super Saturday must be FULLY potty trained. This means being able to voice when they need to use the restroom, get on and off the toilet by themselves, wipe by themselves, and dress by themselves. Super Saturdays are for kiddos 4 years and up! We are no longer offer Fun Friday events. Open Gym and P.O.P Policy: You must have a ChampionsTX account in order for your children to participate in any open gym or preschool open play. Open Gym and P.O.P is free for students enrolled in classes.

Champions Academy, Inc. Late Fee Policy New policy as of January 1st, 2025: A late fee of $50 will be added to any account not paid by the 7th for 1st time offenses. The late fee will increase by $25 each week the balance is not paid. For second time offenses a $100 late fee will be added to any account not paid by the 7th. The late fee will increase by $25 each week the balance is not paid. If a credit card is expired, you will have the 7-day grace period before being charged the late fee. It is the parent’s responsibility to keep their online account up to date with credit cards, emails, and phone numbers. Please make sure the credit card information and email address on file for your account is correct. Any bank charges or fees generated by the use of a debit card will be the sole responsibility of the customer.

Champions Academy, Inc. Billing Authorization & Financial Policies At ChampionsTX, we strive to make our billing and enrollment process clear and simple. Please review the following information carefully. By enrolling, you authorize ChampionsTX to bill your account according to the policies outlined below. TOTAL DUE AT SIGN-UP At the time of enrollment, the following charges are due: $40 Annual Membership Fee (per student, billed once per year on your anniversary date) Team Fee (for Show Teams, CCA Squads, and Xcel—varies by program) Prorated First Month Tuition (based on your program and start date) Summer Enrollment: To hold your child’s spot for fall classes, only half-month August tuition and your annual membership fee (if due) are required during summer registration. The school-year session runs August through May. FREE TRIAL CLASS We want to help every child find the perfect class fit! While all our classes follow the same skill progressions, each group has its own unique energy and mix of students. Free trial classes are limited to one per child per season. Trials must be scheduled in advance via the Parent Portal, by email, or by phone. Trials cannot be scheduled in full classes or classes with a waitlist. Evaluations with a floor manager are available to help you find the best class placement. DISCOUNTS Sibling Discount 10% off tuition for each additional sibling enrolled. Contact the office to help set up your enrollment and apply your discount! FINANCIAL POLICIES AutoPay Monthly tuition is automatically charged on the 1st of each month. (Example: January tuition is charged on January 1st.) If the 1st falls on a weekend or holiday, payments will process on the next business day. Payments are prepared 5 business days before the charge date and may take several days to fully post. Due to software limitations, payments cannot be split between two payment methods. Families may log in to their Parent Portal at any time to make additional or early payments. ChampionsTX does not mail statements; families are responsible for keeping accounts current and accurate. SESSION INFORMATION Enrollment continues monthly through the school year (August–May) unless written notice of cancellation is provided. Monthly tuition remains the same regardless of 3-, 4-, or 5-week months. Tuition is not prorated for holidays or absences. New students enrolling mid-month will have tuition prorated at sign-up. ChampionsTX does not issue refunds, but credits may be applied to future programs when applicable. ANNUAL MEMBERSHIP FEE $40 per student, due upon enrollment and renewed annually on your anniversary date. Non-refundable and non-transferable. Covers insurance, administrative processing, and correspondence. All children need to have an active membership/ Anniversary Fee to participate in Classes, Pop, Camp, Super Saturdays , and anything that we offer here at ChampionsTX. PROGRAM & EVENT POLICIES Camp Cancellation Policy Our camps are carefully staffed and supplied based on enrollment numbers. Payment is due in full at registration and is non-refundable. Reschedule requests received 7+ days in advance may be accommodated if space allows. Illness: With a doctor’s note, camps may be transferred to another date. COVID: Families may transfer to a future camp week if illness occurs in the household. Transfers requested less than 7 days in advance will incur a $35 transfer fee and one day’s tuition charge. Cancellations: 14+ days before camp = full credit 7–13 days before camp = 50% credit

Champions Academy, Inc. General Authorization and Liability Waiver and Release Program Participation I desire for my child(ren) to participate in activities provided by the West Austin Youth Association (WAYA) and Champions Academy, Inc. (ChampionsTX). I understand that during such activities, parents, guardians, siblings, and caretakers/nannies may also be present and may enter the gym floor or activity area for the purpose of observation, assistance, or interaction. Risks I understand the nature of the physical demands involved in these activities and acknowledge that participation or presence in the gym or activity area involves inherent risks, including but not limited to falls, collisions, and other accidents that may result in injury. I understand that such injury may be severe and that these risks apply not only to enrolled participants but also to any parent, guardian, sibling, caretaker, or other accompanying person who enters the gym floor or activity space. I have made WAYA and ChampionsTX aware of any medical or physical conditions that might affect participation by my child(ren) or any accompanying family member or caretaker. I understand that reasonable safety procedures are followed but that unforeseen circumstances or accidental events may occur, for which WAYA and ChampionsTX, their officers, agents, representatives, coaches, volunteers, and employees cannot be held responsible. I hereby assume all ordinary and extraordinary risks normally incidental to these activities and programs, including those which may not be foreseeable. I, the undersigned parent or guardian, authorize the staff of ChampionsTX to make appropriate decisions regarding medical assistance in the event of an accident or illness during my absence. Release I unconditionally waive and release the West Austin Youth Association and Champions Academy, Inc., along with their officers, agents, representatives, coaches, volunteers, and employees, from any and all claims, rights, or causes of action which may be asserted by me, my child(ren), or any accompanying parent, guardian, sibling, or caretaker as a result of any injuries, expenses, or losses—whether physical, emotional, or financial- sustained while participating in or being present during any activities, or while on the premises of WAYA or ChampionsTX. This includes any claims resulting from negligence, acts, or failures to act by WAYA or ChampionsTX and their agents or representatives. Indemnification I agree to indemnify and hold harmless WAYA and ChampionsTX, including their officers, agents, representatives, coaches, volunteers, and employees, from any and all claims, demands, or causes of action brought by any person as a result of injuries, damages, or losses sustained by me, my child(ren), or any accompanying family member or caretaker while on the premises or participating in activities. This indemnification includes all related attorney’s fees, settlements, and litigation costs. No Liability Insurance I understand and acknowledge that WAYA and ChampionsTX do not provide liability insurance for any participant, parent, guardian, sibling, or caretaker. I acknowledge that no member of my family or household is covered by any policy of liability insurance through WAYA or ChampionsTX. Consideration I acknowledge that the consideration for this release is the opportunity for my child(ren), and by extension any accompanying adult or minor, to participate in or be present for activities at WAYA and ChampionsTX. Binding Effect This agreement is binding upon me, my spouse, heirs, assigns, dependents, personal representatives, and estates. It is also binding upon my child(ren), and any accompanying parent, guardian, sibling, or caretaker, as well as any legal guardian thereof. Entire Agreement This document constitutes the entire agreement between WAYA/ChampionsTX and the undersigned regarding the matters covered herein. All prior agreements, oral or written, are superseded. No additional oral or written representations, promises, or assurances exist beyond what is expressly stated in this document. Photo / Video Release I grant permission for ChampionsTX to take instructional and promotional photos or videos of my child(ren), myself, and any accompanying family member or caretaker while in the gym or activity area. I authorize ChampionsTX to use such images or videos for marketing and community purposes, including but not limited to websites, printed materials, and social media. I understand that my child’s or my first name may appear in connection with these images and that I will not receive compensation. All images and recordings remain the sole property of ChampionsTX. If you do not wish to grant photo or video permission, please complete the separate Photo/Video Opt-Out Form found in the Student Policies or request one by emailing WAYA@ChampionsTX.com. Acknowledgment By signing below (or completing digital registration), I acknowledge that I have read, understood, and voluntarily agree to the terms and conditions of this General Authorization / Liability Waiver and Release on behalf of myself, my child(ren), and any accompanying family members or caretakers.

Champions Academy, Inc. Parental Authorization / Liability Waiver and Release Program Participation I desire for my child(ren) to participate in activities provided by the West Austin Youth Association (WAYA) and Champions Academy, Inc. (ChampionsTX). I understand that participation may include physical activity, gymnastics, cheer, tumbling, parkour, and other movement-based programs. Assumption of Risk I understand that the physical nature of these activities carries inherent risks, including but not limited to falls, collisions, and other accidental injuries that may be severe. I have disclosed to WAYA and ChampionsTX any medical or physical conditions that may affect my child(ren)’s participation. I understand that while reasonable safety procedures are followed, unforeseen circumstances and accidental events may occur, and that WAYA and ChampionsTX, their officers, agents, representatives, coaches, volunteers, and employees cannot be held responsible for such events. I voluntarily assume all ordinary and extraordinary risks, whether foreseen or unforeseen, associated with participation in ChampionsTX programs and related activities, including those risks associated with parents, guardians, siblings, or caretakers who may enter the gym floor or activity area. I authorize ChampionsTX staff to make decisions regarding emergency medical assistance if I cannot be reached in the event of illness or injury. Release of Liability I hereby unconditionally waive, release, and discharge the West Austin Youth Association and Champions Academy, Inc. (ChampionsTX), along with their officers, agents, representatives, coaches, volunteers, and employees, from any and all claims, demands, rights, or causes of action that may arise as a result of: Participation by my child(ren) or any accompanying adult or sibling, The use of ChampionsTX or WAYA facilities, equipment, or instruction, or Any act of negligence or omission by the above-named parties. This includes but is not limited to claims for injuries, expenses, loss of compensation, or damages of any kind, whether direct or indirect. Indemnification I agree to indemnify and hold harmless WAYA and Champions Academy, Inc., along with their officers, agents, representatives, coaches, volunteers, and employees, from any and all claims, demands, or causes of action brought by any person as a result of injury, loss, or damage related to my child(ren)’s participation or the presence of myself, family members, or caretakers at the facility. This indemnification includes all associated costs, such as attorney’s fees, settlements, and litigation expenses. Notice of No Liability Insurance I understand and acknowledge that WAYA and ChampionsTX do not provide liability insurance for participants, parents, guardians, siblings, or caretakers. Neither my child(ren) nor any member of my family is covered under any policy through these organizations. Consideration I acknowledge that the consideration for this release is the opportunity for my child(ren) to participate in ChampionsTX and WAYA programs, classes, and activities. My child’s first name may be used in descriptive text or commentary connected to the image(s). I release ChampionsTX from any claims or compensation related to the use of these images. All images remain the property of ChampionsTX. Acknowledgment By signing below (or by completing online registration), I acknowledge that I have read, understood, and voluntarily agree to the terms of this Parental Authorization, Liability Waiver, and Release on behalf of myself, my child(ren), and any accompanying family members or caretakers.

Photo & Media Release I hereby grant ChampionsTX and the West Austin Youth Association (WAYA) permission to use my likeness and/or my child’s likeness in photographs, videos, or other digital media (“photos”) for any lawful purpose, including but not limited to educational, promotional, or marketing materials- both in print and web-based publications- without payment or other consideration. I understand and agree that all photos, videos, and recordings will become the property of ChampionsTX and WAYAand will not be returned. I hereby irrevocably authorize ChampionsTX and WAYA to edit, alter, copy, exhibit, publish, or distribute these photos in any form or media now known or later developed. I waive the right to inspect or approve the finished product in which my or my child’s likeness appears, and I waive any right to royalties or other compensation arising from or related to the use of these materials. I hereby hold harmless, release, and forever discharge ChampionsTX and WAYA, along with their officers, employees, agents, and representatives, from any and all claims, demands, or causes of action that I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or my estate may have by reason of this authorization. Acknowledgment I have read and fully understand the terms of this Photo & Media Release. I voluntarily agree to its terms on behalf of myself, my child(ren), and any accompanying family members or caretakers.

Champions Academy, Inc. Anti-Bullying Policy At ChampionsTX, we believe every child and family deserves to be part of a safe, supportive, and uplifting community. Bullying has no place in our programs, and we are committed to maintaining an environment where all athletes, coaches, staff, and visitors feel respected, valued, and protected. This policy defines bullying, outlines how to report it, and details how ChampionsTX will respond to ensure a positive experience for all. 1. Definition of Bullying Bullying is any intentional, repeated, and hurtful behavior that causes physical or emotional harm, distress, or intimidation to another person. It may include: Verbal bullying: name-calling, teasing, mocking, spreading rumors, or threats. Physical bullying: hitting, pushing, tripping, or unwanted contact. Social bullying: excluding others, encouraging others to isolate someone, or using social influence to harm. Cyberbullying: sending or posting hurtful, threatening, or inappropriate messages or images online or via text. All forms of bullying are unacceptable- whether between athletes, coaches, staff, parents, or visitors. 2. Prohibited Behaviors Bullying of any kind- verbal, physical, social, or digital- is strictly prohibited on ChampionsTX property, at off-site events, during classes, team practices, camps, performances, or any other affiliated activity. Any act that undermines the safety, confidence, or dignity of another person will not be tolerated. Joking, teasing, or sarcasm that targets another person’s appearance, skill level, background, or identity is considered inappropriate and will be addressed. 3. Reporting Bullying Who can report: Any athlete, parent, coach, or staff member who experiences, witnesses, or suspects bullying should report it immediately. How to report: Speak directly to a coach, program director, or manager. Submit a written or electronic report. Anonymous reports will also be accepted. Reporting information and contact details will be displayed in the facility and included in parent communications. All reports will be taken seriously and addressed promptly. 4. Confidentiality ChampionsTX will handle all reports with care and confidentiality, to the extent permitted by law. Only individuals involved in investigating or resolving the incident will have access to information. Protecting the privacy and safety of the children involved is our highest priority. 5. Investigation and Response Once a report is received, ChampionsTX management will conduct a thorough, fair, and impartial investigation. Both the individual reporting and the alleged party will be given an opportunity to share their perspective. Parents or guardians will be notified when minors are involved. Confirmed bullying behavior will result in immediate corrective action, which may include disciplinary measures, suspension, or expulsion. If deemed necessary, law enforcement or child protection agencies will be contacted. 6. Non-Retaliation ChampionsTX strictly prohibits retaliation or intimidation against anyone who reports bullying, participates in an investigation, or supports another person in doing so. Any form of retaliation will lead to disciplinary consequences. 7. Support and Restoration We recognize that both victims and those engaging in bullying behavior may need guidance and support. ChampionsTX will provide access to supportive resources such as: Counseling referrals Restorative conversations and mediation (when appropriate) Ongoing follow-up to ensure the environment remains safe and positive Our goal is not only to stop bullying but to restore trust, safety, and connection within our community. 8. Prevention and Education ChampionsTX promotes proactive prevention through education, positive role modeling, and clear expectations. Coaches and staff receive annual training on identifying, preventing, and addressing bullying behaviors. Athletes are taught the importance of kindness, teamwork, inclusion, and speaking up for themselves and others. Parents are encouraged to reinforce these values at home and partner with staff in promoting a healthy culture. 9. Consequences Any individual- whether athlete, parent, coach, or visitor- who engages in bullying behavior will face disciplinary action, which may include: Verbal or written warning Suspension from class or team activities Permanent dismissal from ChampionsTX programs or facility access The severity of the consequence will reflect the nature and impact of the behavior. 10. Policy Review This policy will be reviewed annually and updated as necessary to reflect evolving best practices and maintain compliance with Texas laws and youth safety standards. Our Commitment By upholding this Anti-Bullying Policy, we ensure ChampionsTX remains a place where every athlete can grow in confidence, character, and community- free from fear or harm. Sexual Abuse and Harassment Prevention Policy At ChampionsTX, we are committed to providing a safe, supportive, and respectful environment for every child, family, coach, staff member, and visitor. This policy outlines our commitment to the prevention, reporting, and response of any form of sexual abuse, harassment, or misconduct within our programs and facilities. 1. Zero Tolerance ChampionsTX maintains a zero-tolerance policy for any form of sexual abuse, harassment, or misconduct. This includes behavior toward athletes, staff, parents, volunteers, or visitors- by any individual associated with our programs. 2. Definitions Sexual Abuse: Any unwanted sexual contact, conduct, or behavior- including physical contact, verbal comments, gestures, written communication, or electronic messages- that violates another person’s boundaries or creates an unsafe or intimidating environment. This includes but is not limited to: inappropriate touching, sexual advances, grooming behaviors, or sharing sexually explicit material. Sexual Harassment: Unwelcome sexual advances, requests for sexual favors, or verbal or physical conduct of a sexual nature that interferes with participation, employment, or the enjoyment of ChampionsTX activities or programs. 3. Reporting Requirements Who must report: Any staff member, coach, volunteer, athlete, parent, or visitor who witnesses, suspects, or experiences sexual abuse or harassment must immediately report it to ChampionsTX management or the designated Safe Environment Officer. How to report: Reports may be made: In person to a manager, program director, or owner In writing (email or letter) Anonymously, if preferred Reporting information and contact details will be posted prominently in the facility and available online. Mandatory reporting: In accordance with Texas law, any suspicion of abuse involving a minor will be reported immediately to Child Protective Services (CPS) or local law enforcement. 4. Confidentiality All reports will be handled with strict confidentiality to the extent permitted by law. Only individuals directly involved in the investigation and resolution process will have access to details of the report. 5. Investigation and Response All reports will be promptly and objectively investigated by management or an appropriate third party. Appropriate corrective actions will be taken, which may include suspension, termination, or permanent removal from the facility. In cases involving potential criminal activity, law enforcement will be contacted immediately. Parents or guardians of affected minors will be notified as appropriate. 6. Non-Retaliation ChampionsTX strictly prohibits retaliation against anyone who, in good faith: Reports sexual abuse or harassment Participates in an investigation Supports another individual involved in the reporting process Any retaliatory behavior will result in disciplinary action up to and including termination or expulsion. 7. Support and Resources ChampionsTX is committed to providing support to individuals affected by abuse or harassment. Resources such as counseling referrals or victim advocacy services may be made available upon request. 8. Prevention and Education All staff, coaches, and volunteers will receive annual training on recognizing, preventing, and reporting sexual abuse and harassment. ChampionsTX promotes a culture of accountability and transparency, ensuring all participants understand expectations for professional and appropriate behavior. One-on-one interactions between an adult and a minor are strictly prohibited. All interactions must occur within observable and interruptible settings. Private communications (text, phone, or electronic) between adults and minors must include a parent/guardian or supervisor. 9. Consequences Any individual- staff, volunteer, athlete, parent, or visitor- found to have engaged in sexual abuse, harassment, or misconduct will face immediate disciplinary action, up to and including: Suspension or permanent removal from ChampionsTX programs or facilities Termination of employment or volunteer status Notification of law enforcement as applicable 10. Review and Updates This policy will be reviewed annually and updated as needed to ensure continued effectiveness and alignment with state law, mandated reporter requirements, and youth safety best practices.
